Delivery Instruction Sheet

To apply to become a USDA Afterschool Food Program Site, you must provide all these items to Children’s Hunger Alliance:

Approved Fire Inspection
Approved Health Inspection
Signed Separate Entity Agreement
New Site Approval Visit Form
Signed Waste Policy Agreement
Site Profile
Organizational Chart of Afterschool Program
Copy of 501(c)3

Note: All pathways to delivery destination should be clear. If the driver cannot get through the room, the coolers will be left near the door.
