Fire Inspection Guidelines

To apply to become a USDA Afterschool Food Program Site, you must provide all these items to Children’s Hunger Alliance:

Approved Fire Inspection
Approved Health Inspection
Signed Separate Entity Agreement
New Site Approval Visit Form
Signed Waste Policy Agreement
Site Profile
Organizational Chart of Afterschool Program
Copy of 501(c)3
Delivery Instruction Sheet

All Children's Hunger Alliance USDA Afterschool Food Programs must be able to provide the following:

  • Provide an annual inspection report of the fire alarm system.
  • Provide an annual inspection report of the sprinkler system.
  • All fire extinguishers must display an inspection tag dated within the last 12 months.
  • If room in which the program is conducted can hold 50 or more people – the site must obtain a Place of Assembly Permit and display a room capacity sign. (information can be obtained by calling Inspector Rickman @ 645-7641 x5622.)
  • All sites must have displayed an evacuation floor plan drawing using red arrows as evacuation route in each room where the afterschool program will be held.
  • All sites must have a written evacuation procedure.
  • All sites must have a written fire drill log displaying the following information:
    • Date of fire drill
    • Time of fire drill
    • How long it took to evacuate (needs to be under 3 minutes)
    • Total number of children
    • Total number of adults/staff
    • Method of alarm – (verbal or did you use the alarm system)
    • Comment section
    • Initial of person(s) who conducted the fire drill
